Toddlers To Grandparents is looking for compassionate, organized, and dedicated Service Facilitators to join our growing team.
As a Service Facilitator, you will help individuals and families who receive Medicaid consumer-directed services, manage their in-home care. You will act as a guide making sure families understand the program/waiver they have, helping them hire and manage their own caregivers, keeping their case records up-to-date, and ensuring Medicaid rules are followed. Holding a caseload starting at 30 people and growing, you’ll visit families in their homes, check that care needs are being met, offer resources, and provide support when challenges arise. It’s a role that blends community connection, problem-solving, and advocacy — Every day, you’ll be making a difference!
